This is a sample of a reference letter for use upon the initial application for a license.
It is the applicant’s responsibility to obtain three letters of reference.
The letters of reference are required for the initial application and must be sent to the County Social Service Agency. A copy of each reference letter must be sent to the Human Service Center.
This form may be duplicated.
